  • Your old and new addresses, including postcodes
  • Your account number, if you're an existing Phoenix Supply customer
  • Meter reading(s) for your current premises and if possible for the premises that you are moving into
  • Your meter serial number
  • Your moving out and moving in dates
  • A contact telephone number
